Leaving or joining National Health Insurance in Takasaki

If you need to leave or join National Health Insurance (NHI) in Takasaki because of leaving a job, starting a new job, moving in or out of Takasaki or another reason, you need to inform the city within 14 days of the change.

Table of procedures and documents required in each situation

Starting a job where health insurance is provided (and therefore no longer need NHI)
Became a dependent of someone with health insurance provided through their job (and therefore no longer need NHI)
NHI inssued health insurance card of the person concerned
New health insurance card issued by the employer
A dependent student who was living outside Takasaki City but using a Takasaki NHI health insurance card stops being a student (because of graduation or leaving studies, etc.) NHI issued student health insurance card (shugakuyo kokumin kenko hokensho)
Other reason Change of address, change in head of household, change of name, etc. NHI issued health insurance card

Lost your NHI issued health insurance card Photo ID※

If a proxy outside the person's immediate family is reporting the change on their behalf (dairi-nin), they also need to provide a signed declaration giving them approval to do so (inin-jo).

※In order to be issued the new card at the counter on the day of reporting the card's loss, you must be able to provide photo ID such as your Zairyu Card (if a proxy is the person reporting the loss, the new card will be mailed out at a later date).
