National Health Insurance cards will be discontinued from December 1st, 2024. Instead, please use your My Number card and register it as your health insurance card using the My Number portal. This allows you to combine your My Number and health insurance cards into one. This is called My Number insurance card (or maina hoken-sho in Japanese).
If you allow it, clinics can use the My Number insurance card to see your medical history and any medications you have been prescribed, to help provide even better care than before.
Takasaki City sent a notice in the mail to all residents who are enrolled in national health insurance as of August 1st, 2024. This notice contains personal information on any household member enrolled in national health insurance. It will also contain the last four digits of your individual number (your My Number). Please check that the information written there is correct.
In the unlikely event that the information is incorrect, please contact the Health Insurance & Pension Division.
