Applying for a My Number Card

A My Number Card is easy to make and can be used as a photo ID. 

The application process

Do you have the application paper? It is an A4 sized paper that was included with the green paper version of the My Number Card sent in September. If you do not have the application paper, skip down to the "I don't have the application paper" section for instructions on what you should do.

I have the application paper

Applying by mail:

　(1) Fill out the application paper and glue an acceptable photo onto it
　(2) Use the return envelope that was provided with the application paper and mail it in. You can also download the envelope from here＜外部リンク＞ (Japanese only)

Applying online:

　(1) Use a smartphone or other device to take your picture

I don't have the application paper

Bring a photo ID to the Citizens Division located on the 1st floor of Takasaki City Hall (or the Welfare Division of any branch location). Then another copy of the application paper can be made for you. Either you or another member of your household can request another copy. If you need the return envelope printed as well be sure to ask.

It takes one to two months to receive a My Number Card. When the card is ready for pickup, you will receive a notice in the mail. You need to bring that notice and a photo ID to City Hall (or any branch location) in order to receive the card.

What can I use a My Number Card for?

• Official photo ID
• To apply to file taxes online
• To receive documents such as a residence certificate copy (and others) from the convenience store
• In some cases, it can be used at the hospital in place of a health insurance card (*it depends on the hospital)
