If you need to leave or join National Health Insurance (NHI) in Takasaki because of leaving a job, starting a new job, moving in or out of Takasaki or another reason, you need to inform the city within 14 days of the change.

If a proxy outside the person's immediate family is reporting the change on their behalf (called dairi-nin in Japanese), they also need to provide a signed declaration giving them approval to do so (this is called a inin-jo).
※In order to have the shikaku kakuninsho or joho no oshirase notices reissued on the same day, you must be able to provide a photo ID such as your residence card (if a proxy is reporting the loss, the new notice will be mailed out at a later date).
